When he stood up, he struck his head on the bottom of the bucket, lost his balance, and fell approximately four feet off the side of the truck to the ground breaking the lower left leg.

An employee was loading a well with hot oil. The employee had turned valves off and started to fill the well. The pressure in the tank filled due to the hot oil. The pressure release valve released flammable vapors and/or gases. The flammable vapors and/or gases ignited, burning the employee's face (cheeks and ears). The employee was wearing flame resistant clothing, a hard hat, and safety glasses at the time.
